Chronic Care Management

Services for managing various, typically two or more chronic diseases that are projected to persist for about a year or until the patient’s demise are known as chronic care management services (CCM).

These services are typically delivered to Medicare beneficiaries who are non-face-to-face. The care team may bill for the time spent controlling patients’ circumstances under CCM.

CCM in healthcare includes creating a thorough care plan, engaging remote management and communication (often over the phone), managing medications, and coordinating care across doctors.

The idea behind chronic care management services was to provide a means of payment for medical professionals who treated patients beyond the typical parameters of an office visit.

Coincidentally, the implementation of CCM resulted in a more effective method for care teams to actively engage and manage patients with challenging chronic conditions, increasing outcomes and lowering treatment costs.
